As a Case Manager you will meet with older adults and people with disabilities to assess their needs, educate them about available services and assist them in securing the services they need. Position Responsibilities : Conduct in-home assessments of the consumer's overall situation to determine needs to create and implement person-centered care plans including identifying, coordinating, authorizing, securing and monitoring community based services on an ongoing basis. Modify and adjust care plans as the consumer's needs or situation changes. Qualifications Experience in the human services field or Bachelor's degree desired Bilingual Haitian Creole preferred Proficient computer skills Expert time management Strong desire to help others with the ability to establish positive and professional relationships Advocate for those needing services and assistance A sincere desire to help others Detail oriented The ability to appropriately handle confidential and personal information Reliable transportation with the ability to make home visits throughout the GLSS service area Hours Full Time 40 hours per week (Monday-Friday) *4 day work week available *GLSS is an affirmative action and equal opportunity employer.
