Job Title: RN - Home Health

Location: Le Mars, IA 51031

Shift: Day shifts (M - F) with one in four call rotation for nights and weekends.

Travel Pay Rate: $1950 weekly ($22/hr on w2 + $1246/week stipend) - (32 hours)

Local Pay Rate: $53.15 per hour on W2 - (32 hours)

Start Date: 11/17/2025

Job Type: 13-week Contract

Position Summary:

• The qualified candidate will have 2 years of experience performing OASIS assessments and care planning for in-home patients.
• BLS certification required
• This person will also be in the call rotation for the department, one in four.
• Scheduled shifts are days, Monday through Friday.
• We are open to 32-40 hours per week.
• Facility is in need for a Home Health RN who will be responsible for the care of patients in their home.
• Our service area is Plymouth County, Iowa.
• Previous experience with our EMR, Meditech, is a plus.

Key Responsibilities:

• Conduct thorough patient assessments to develop individualized care plans tailored to each patient's needs.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols and best practices.
• Monitor patients' vital signs and overall health status, documenting changes and communicating effectively with the healthcare team.
• Provide education and support to patients and their families regarding health management, disease prevention, and wellness strategies.
• Collaborate with interdisciplinary teams to coordinate care and ensure continuity across various healthcare settings.
• Manage medical equipment and supplies required for patient care, including IV infusion therapy and wound care.
• Maintain accurate medical documentation in compliance with HIPAA regulations and organizational policies.
• Assist with activities of daily living (ADLs) for patients requiring support due to illness or disability.
• Participate in case management activities, including discharge planning and follow-up assessments.

Required Qualifications:

• RN IA State or Compact License & BLS from (AHA)
