Job Description:

Phoenix Home Care & Hospice is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide compassionate, skilled 1:1 nursing care in the homes of our clients in Joplin, MO . We have a variety of shift options available, including day, evening, and overnight shifts , along with shift premiums for weekend and overnight shifts.

Job Responsibilities:

• Deliver care based on the individualized plan of care developed with the physician
• Monitor, document, and report the patient’s condition using electronic charting
• Educate and provide support to the client and their family
• Attend training sessions and team meetings as required

Qualifications:

• Active State of Missouri or Compact Nursing License (LPN or RN)
• Strong communication and critical thinking skills
• Compassionate, professional, and positive attitude
• Ability to work independently and manage time effectively
