The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime. Minimum Requirements: + Current RN licensure in state practicing + At least one year of Case Management experience preferred + Current CPR if applicable + TB questionnaire, PPD or chest x-ray if applicable + Current Health certificate (per contract or state regulation) + Must meet all federal, state and local requirements + Must be at least 18 years of age ?? This role is ideal for nurses who thrive in fast-paced hospital settings and are passionate about coordinating care across the continuum. As an Inpatient Case Manager, you'll assess, plan, implement, monitor, and evaluate individualized care plans. You'll collaborate with social services and interdisciplinary teams to ensure optimal patient outcomes while managing resources effectively. Key Responsibilities: + Daily patient assessments and reassessments + Strategic planning with care teams + Coordination of services across departments + Use of critical thinking and conflict resolution skills + Goal setting and outcome measurement + Strong documentation and organizational skills Requirements: ? Active California RN License ? Minimum 2 years of Case Management experience ? Strong clinical judgment and communication skills ? Ability to work some weekends (mostly Saturdays) ?
