Horizon Home Care and Hospice is seeking Weekend Visit Nurses to join our compassionate and patient-centered Hospice Team.

As a Baylor (Weekend Visit Nurse) RN you will serve a valuable role in providing loving continuity on weekends for our hospice patients who require weekend visits or are experiencing a change in symptoms. Baylor nurses have ample time to support the patients and families they visit on weekends while allowing them the week days to be at home.

Baylor nurses:

• Assess patient symptoms and family / caregiver's situation and provide appropriate clinical intervention with the supportive hospice team for those patients needing a weekend visit.
• Identify and provide for patient and family's educational needs..
• Have Agency provided laptops, cell phones, portable printers, bag, Fleet car PRN
• Communicate patient's issues and concerns to the Care Manager / Patient Care Coordinator, appropriate health care professional, IDG (Hospice only) and / or Clinical Manager

#nursing

Degree from an Accredited School of Nursing (BSN preferred)

Current WI RN license

1-2 years of Hospice, Oncology or related experience

Experience in the care of the terminally ill; knowledge of Hospice Care

Hospice certification is preferred

Current/Valid WI Drivers License; plus reliable/insured vehicle
