Job Description

New Age Hospice is seeking a Hospice Licensed Social Worker-PRN in the Houston, TX and surrounding areas.

Job Duties

  • Territory includes, Houston and surrounding areas
  • Provide individual and family counseling in home
  • Complete relevant documentation
  • Consult with hospice team (IDT)
  • Participate in IDT
  • Develop care plans
  • Provide resources to assist patient and families in the community
  • Provide PASSR information and coordinate respite care
  • Application assistance to Medicaid and other insurance providers
  • Provide case management, advocacy, resource linkage as needed
  • Provide visits as directed to include admission, recertification, and care plan necessitated
  • Maintain professional relationship with staff and referral sources

Requirements

  • Master's degree in social work and associated licensure - MSW
  • 1 year experience preferred working as social worker with hospice or home health care.
  • Ability to work in a professional manner
  • Must work well both independently and in a group setting
