JOB SUMMARY:

The Psych Rehab provider is responsible for engaging and encouraging clients to meet their chosen goals. Psych Rehab can support clients with education/employment opportunities while also providing clients with a sense of belonging, supportive relationships, valued roles and community. The Psych Rehab provider’s goal is to promote wellness, independent living, self-direction and recovery; and to enhance the skill and ability of clients to meet their goals.

Assures adherence to state statutes and rules, CARF standards, and federal regulations including HIPAA and 42 CFR Part 2. Positively represents the organization and promotes Rimrock Foundation’s Mission to the community.

Essential Functions

  • Contribute to the development and execution of person-centered care plan, including assisting the client to understand the plan and instructions, and tailor communication to appropriate health literacy levels.
  • Work with client to assess client’s readiness to change, assist client in making changes to daily routines, and identify barriers; assist client in developing strategies to address barriers.
  • Identify available community-based resources.
  • Collaborate with Care Team/Care Coordinator to help client access community-based resources, engage in services, and follow up with process of goals.
  • Work to build a therapeutic alliance with client in order to bolster effectiveness of services/support.
  • Document client’s progress via progress notes within 24-hours.
  • Be knowledgeable in a variety of topics such as: health/wellness, employment/education, community resources, daily living skills, implementing coping skills, etc., as they may related to rehabilitation and recovery goals.

Experience Requirements

  • Possess a minimum of one (1) year experience working with consumers with a serious and persistent mental illness (preferred) and/or substance use diagnoses.
  • Have demonstrated leadership in maintaining high standards of professional behavior for self and staff; Have organizational, problem solving, creative and independent thinking skills.
  • Have demonstrated cultural competency and a commitment to diversity management.

Qualifications

  • Bachelor's Degree from an accredited college in a Human Services, Social Work, Education or related field or 3 years equivalent work experience. A master's degree in Human Services preferred. Meet all licensing requirements set forth by the respective state of operation.
  • Possess a valid, regular (non-probationary) state driver's license if the individual will be driving either their personal vehicle for on-the-job purposes or any agency vehicles.
  • Successfully pass an initial medical examination and pass a drug screening.

  • Adapts philosophy of person-centered, recovery-oriented care.
  • Lifelong learner who takes the initiative and is motivated to learn new things in order to teach clients various strategies to achieve their goals.

Physical Demands

Work is indoors in an office environment with moderate noise. Intermittent physical effort involving lifting of up to 25 pounds, walking and stooping is required.

WORK ENVIRONMENT/JOB LOCATIONS:

Involves everyday risk or discomfort requiring normal safety precautions in an office setting, meeting room and patient care areas. Work may involve mental and emotional stress. Some travel required.
