Job Description The Opportunity:As a Hospice Nurse at Saint Luke's Hospice House, you will provide end of life care and symptoms management to patients straight from the ICU outside their home. Many of these patients require aggressive symptom management and may be on drips, LVADs, ventilator/bipap, or have trachs. You will provide patient education to patients and patients' loved ones, provide medication administration. You will work with a multidisciplinary team in the care management plan and use a team approach to provide exceptional patient care.Shift: Full-time, 3 12-hour shifts per week, nights.The Work:Provides direct nursing care to patients with terminal illness. Management and coordination of care to include both the patient and family. Completes initial assessment of physical, emotional, and spiritual status. Aggressive pain and symptom management, including frequent medication administration and adjustments. Management of IV infusions and titration of drips.Provides education and support related to end of life and imminent death. Open, sensitive, and non-judgmental presence accepting of different beliefs, cultures, and values.
